On April 9, 2014, a 911 call-routing facility in Englewood, CO, stopped directing emergency calls to many 911 call centers (Public Safety Answering Points or PSAPs) in 7 states -- Calif., Florida, Minnesota, N. and S. Carolina, Pennsylvania, and Washington. The outage was caused by a software coding error in the Colorado facility, and resulted in a loss of 911 service for more than 11 million people for up to 6 hours. Over 6,600 calls to 911 never reached a PSAP. What is most troubling is that this is not an isolated incident or an act of nature. So-called šůžsunny dayšůĚ outages are on the rise because the interaction of new and old systems is introducing fragility into the communications systems. This report of the investigation of the causes and effects of the April 2014 outage includes recommendations for maintaining a reliable 911 system. Tables and figures. This is a print on demand report.
