Yarnell Hill Fire (June 20, 2013): Serious Accident Investigative Report

This report analyzes the circumstances leading to the June 30, 2013 entrapment and deaths in central Arizona of 19 firefighters of the Granite Mountain Interagency Hotshot Crew, (IHC), hosted by the Prescott Fire Department. They were fighting the Yarnell Hill Fire and died after deploying fire shelters. One crewmember was separated from the crew earlier that day and was not at the deployment site. This report has two parts. Part One includes the fact-based Narrative of the incident and offers the investigative teamäó»s Analysis, Conclusions, and Recommendations. Part Two, the Discussion section, is meant to prompt discussion and facilitate learning, and aims to support the broader community seeking to make sense of the accident and to improve safety and resilience.
