Controlled Collision with Terrain Transportes Aereos Ejecutivos, S.A. (TAESA) Learjet 25D, XA-BBA Dulles Internat. Airport, Chantilly, VA, 6/18/94
Barry Leonard (ed.). Explains the accident involving the TAESA Learjet 25D that crashed near the threshold of runway 1R at Dulles Internat. Airport, Chantilly, VA, on June 18,1994. Safety issues included weather at the airport, flightcrew training, qualifications, & performance, flightcrew fatigue, operations specifications, passenger seating, & the ground proximity warning system. The probable causes of the accident were poor decisionmaking, poor airmanship, & relative inexperience of the captain in initiating & continuing an unstabilized instrument approach that led to a descent below the authorized altitude without visual contact with the runway environment.
